Coordination With Clinical Providers

At Anchortees Health Inc., coordination with clinical providers is a core part of our Psychiatric Rehabilitation Program (PRP). We work closely with therapists, psychiatrists, primary care physicians, and other healthcare professionals to ensure every client receives consistent, integrated, and effective care.

How We Coordinate Care
  • Communication With Therapists & Psychiatrists Our PRP team maintains regular communication with each client’s clinical providers to support treatment goals, medication plans, and therapeutic progress.
  • Shared Treatment Planning We align our Individual Rehabilitation Plans (IRPs) with the client’s clinical treatment plan to ensure all services work together toward the same outcomes.
  • Progress Updates & Collaboration With client consent, we share relevant updates, observations, and progress notes to help clinical providers make informed decisions about care.
  • Appointment Support PRP staff assist clients with scheduling, reminders, transportation planning, and follow through for therapy, psychiatry, and medical appointments.

    Crisis & Safety Coordination If concerns arise, our team collaborates with clinical providers to ensure timely intervention, safety planning, and continuity of support.
  • Whole Person Care Approach We believe mental health is connected to physical, emotional, and social wellbeing. Our coordination ensures clients receive comprehensive support across all areas of their lives.
