Job Title: Rehabilitation Specialist / Counselor (PRP)

Position Overview
Anchortees Health Inc. is seeking a skilled and compassionate Rehabilitation Specialist / Counselor to support individuals enrolled in our Psychiatric Rehabilitation Program (PRP). This role focuses on delivering structured, recovery-oriented services that help individuals develop the skills necessary for independent living, emotional stability, and community integration.
The ideal candidate combines clinical insight with practical, hands-on support to drive measurable client progress.
Key Responsibilities
- Provide individual and group-based rehabilitation services aligned with Individualized Service Plans (ISPs)
- Conduct assessments and contribute to service planning
- Deliver skill-building interventions (coping skills, daily living skills, social functioning)
- Provide supportive counseling and psychoeducation
- Monitor client progress and adjust interventions as needed
- Maintain accurate and timely documentation in compliance with Medicaid/DMAS standards
- Collaborate with PRP staff, QMHPs, case managers, and external providers
- Support clients with community integration and resource access
Qualifications
- Bachelor’s degree in Psychology, Social Work, Counseling, or related field (Master’s preferred)
- Must meet or be eligible for QMHP (Qualified Mental Health Professional) status in Virginia
- Minimum 2+ years of experience in mental health, counseling, or psychiatric rehabilitation
- Strong understanding of recovery-oriented and person-centered care models
- Excellent communication, engagement, and documentation skills
Preferred Credentials
- Licensed or license-eligible (LPC, LCSW, or equivalent)
- Experience with PRP services and Virginia DMAS requirements
- Training in trauma-informed care and crisis intervention
