Job Title: Direct Care Worker (DCW) / PRP Specialist

Position Overview
Anchortees Health Inc. is seeking compassionate, dependable, and motivated Direct Care Workers (DCWs) to support individuals enrolled in our Psychiatric Rehabilitation Program (PRP). This role focuses on delivering community-based, person-centered services that promote independence, emotional stability, and overall well-being.
DCWs work directly with clients in home and community settings, helping them build practical life skills and achieve individualized recovery goals.
Key Responsibilities
- Provide one-on-one support aligned with individualized service plans (ISPs)
- Assist clients with daily living skills (hygiene, meal prep, budgeting, organization)
- Support medication adherence (reminders and observation only)
- Implement coping strategies and behavioral interventions
- Facilitate community engagement (appointments, social activities, resource access)
- Document services accurately and in compliance with PRP standards
- Collaborate with clinical staff, case managers, and families as needed
Qualifications
- High school diploma or GED required (Bachelor’s degree in human services preferred)
- Experience working with individuals with mental health conditions preferred
- Strong interpersonal, communication, and problem-solving skills
- Ability to work independently in community-based settings
- Reliable transportation and valid driver’s license
- Ability to pass background check and meet state compliance requirements
Preferred Skills
- Knowledge of psychiatric rehabilitation principles
- Crisis intervention and de-escalation techniques
- Familiarity with documentation and service tracking systems
- Cultural competency and trauma-informed care approach
