Position Overview

Anchortees Health Inc. is seeking a detail-oriented and compassionate Case Management Specialist / Representative to support individuals receiving behavioral health and psychiatric rehabilitation services. This role focuses on coordinating care, connecting clients to essential resources, and ensuring continuity of services that promote independence, stability, and overall well-being.

The Case Management Specialist serves as a key liaison between clients, providers, and community resources.


Key Responsibilities

  • Conduct intakes, needs assessments, and service planning
  • Develop and monitor Individualized Service Plans (ISPs)
  • Coordinate services across healthcare providers, social services, and community programs
  • Connect clients to housing, employment, healthcare, and financial resources
  • Advocate for clients to ensure access to appropriate services and supports
  • Maintain accurate, timely documentation in compliance with Medicaid/DMAS standards
  • Monitor client progress and adjust service plans as needed
  • Provide crisis support and referrals when necessary
  • Maintain regular communication with clients, families, and care teams

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred
  • Relevant experience in case management, behavioral health, or social services strongly preferred
  • Knowledge of community resources and support systems
  • Strong organizational, documentation, and time management skills
  • Excellent communication and interpersonal abilities
  • Ability to manage multiple cases effectively
  • Valid driver’s license and reliable transportation
  • Ability to pass background check and meet state compliance requirements

Preferred Credentials

  • QMHP (Qualified Mental Health Professional) eligibility or certification (Virginia)
  • Experience with Medicaid/DMAS documentation and service coordination
  • Familiarity with trauma-informed and person-centered care models
